The Origin Story
Some ideas arrive fully formed. This one had been building for years — inside emergency departments, across clinical shifts, through two decades of watching what happens when people don't have the right guidance at the right time.
Dr. Malcolm Borg is a Consultant in Emergency Medicine — Maltese by origin, trained through the Royal College of Emergency Medicine, and based in Abu Dhabi, where he works at Burjeel Medical City. His clinical career has taken him across high-volume emergency departments leading multidisciplinary teams, redesigning systems, and integrating data analytics into frontline care. But alongside all of that, he had been working on something else entirely.
For years ,Malcolm had been developing a structured system of algorithms in collaboration with health consultants. A framework for translating what professionals understand about the human body into something genuinely personalised for the individual. Not a programme that calls itself adaptive. One that actually is.
By the time the conversation with Threls began, the thinking was already done. And that made this unlike almost any brief we had encountered before.
